If you are using this page for mail in, remember that you need to mail the copy of the page in with your deposit (cashiers check, money order, or credit cards accepted below). NOTE: Filling out the "Application Form" and submitting it will not ensure you spot in the school. We must have your deposit to hold your spot. Enrollment is on a first come first basis.
